What is the Behavior Checklist Maker ?
If there is a student in your classroom or you someone that has a behavior disorder, then the self-check behavior checklist is a great intervention for them. Not only is it beneficial for the student, but it helps the teacher out as well when the students is able to manage their own behavior. The behavior checklist is a free application teacher can use to create a personalized checklist for students to check their own behaviors throughout the day. - CREATE! Click on the 'selected checklist' drop down menu. Then, click on the arrow next to the item you would like to select for your students personalized behavior checklist.
- EDIT! Then you can edit the self-check behavior items in the 'your checklist' box by clicking edit. A separate window will appear where you can make edits and write notes.
- DOWNLOAD/ PRINT! When you are ready to download the checklist, click on the tabs of the report choices below. You can print the checklist or email them to others. You can also add a name by typing in the box labeled 'report title'. This option will appear near the bottom of printing options.
- SAVE! If you create an account on Intervention Central, you can save your work.
Student Profile
In my field experience class, I have a student who has a hard time managing his behavior. In particular, he has a hard time entering the classroom. When he comes in, he will scream "what is going on, what did I miss?" This behavior is unacceptable. Creating a behavior checklist will remind this student how to enter the classroom. He can carry it around with him as he leaves and reenters the classroom. He also has a difficult time managing his behavior when the teacher is talking, and during independent work time. He needs constant reminders to raise his hand and to follow directions quickly. With the self-check behavior checklist, he will have the reminders right there next to him, so the teacher can focus on teacher and he can manage his own behavior by referring to the checklist.

Step 2: As you can see, the image to the left shows what it will look like for step 2. This is where you can edit, delete, or add notes to the behaviors on your checklist (Intervention Central: Self-Check Behavior Checklist Maker).

Step 3: This is the step where you can download, print or email your customized behavior checklist. As the image shows, you can print or download a PDF, print RTF (word document), or email it by entering in the correct email address.

Step 4: If you have an account, the final step would be saving your work. Once you click save, you can change the name.
